If you have whole blood (lavender-top) EDTA or (yellow-top) ACD-A tubes on hand, you can mail in a sample with the downloaded Practitioner Requisition Form today. (Note: Payment must accompany samples unless you have set up a Medical Account.)

Specimen Types
- Blood: 5-10 cc whole blood (lavender-top) EDTA or (yellow-top) ACD-A tubes
- Buccal: Genelex supplied cheek swab kits, blood is preferred as buccals have a small failure rate.
- Turnaround Time: Ten days, (STAT turnaround time: five days)
